PEN51 Information Sharing Documentation form (ISG)

In this instruction you will learn how to access and complete ISG Documentation form.

ISG in an initiative of the South Australian government to promote the safety and wellbeing of children, young people and families. These guidelines provide a legal and practical framework for sharing client information.

In normal information sharing circumstances, where consent has been given and/or sharing is in the context of primary purposes consent, the details are to be recorded in existing Client Case notes in accordance with normal program practice.

The ISG Documentation form must be completed when information is shared without consent or when information sharing requests are refused by you or to you.  This form records all significant decisions and steps under taken such as:

  •  policies and procedures and Risk Management processes followed if
  •  if and how consent was sought
  • reasons for overriding the client’s wishes or for not seeking consent
  • advice received or requested from others (including staff at the Child Abuse Report Line)
  • reasons for not agreeing to an information sharing request
  • specific confidential information that was shared.
